Just seen some very interesting cases of Neuromyelitis Optica. Actually seeing the tissues and the lesions in the flesh as it were is so illuminating. They were pathologically very different from MS, probably more EAE-like in some respects.
In one case there were some classical NMO lesions devoid of astrocytes, but close by there were some demyelinated lesions with no major effect on astrocytes or axons in the spinal cord lesiosns. These demyelinated lesions were like type III white matter lesions that show evidence of hypoxia (lack of oxygen) it was believed that this hypoxia related to swelling and structural constraint perturbations within the spinal cord affecting the local vasculature, but not any mention of stuck values or neck veins.
